Two ways to apply for membership:

1. PRINT APPPLICATION. View and print the the application form*, fill it out, sign it, and mail with payment (or pay Online) to: OVBA, PO Box 252, Eden, UT 84310. Upon approval by the OVBA, your web page will appear on this web site.

2. ONLINE APPLICATION. In order to quickly notify the OVBA and the Webmaster of your intentions, you may use the on-line form. Please be aware that your membership application will not be complete until the OVBA receives a signed copy of your application with payment (or pay Online). Please print and sign the on-line application just before you hit the submit button. Mail it to: OVBA, PO Box 252, Eden, UT 84310.
